Adult Social Care Transformation Programme

We are working with Torbay and South Devon Foundation Trust on an Improvement Plan to improve the way that Adult Social Care is delivered in Torbay

The Adult Social Care (ASC) Transformation Programme is one of four pillars within the wider ASC Programme and sets out how services will improve over time in Torbay.

It brings together changes across operations, commissioning and assurance to create a more joined-up, sustainable and person-centred system.

The programme is being delivered alongside the planned end of the Section 75 agreement and the return of Adult Social Care to the Council by 31 March 2027. This creates an opportunity to simplify ways of working, strengthen accountability and improve how services are delivered.

Adult Social Care in Torbay is facing several challenges including increasing demand for services, financial pressures, complex and fragmented structures across organisations, and inconsistent ways of working.

This programme provides a coordinated approach to improving services, helping ensure they are sustainable, consistent and better focused on the needs of people who use them.  

Transformation Projects and Workstream

ASC Change

  • Operating Model Redesign (by 31 March 2027) - Redesigning team structures, roles and responsibilities to create a clearer and more effective way of working.
  • Integrated Commissioning Arrangements (post 1 April 2027)- Aligning how services are planned, commissioned and funded across the system.
  • Assurance Realignment (post 1 April 2027) - Strengthening oversight, quality assurance and performance management following the transition.

Care Management System (CMS)

Implementing a new digital system for case management, recording and reporting, replacing current systems and improving access to information. Planned go‑live: 1 September 2026. 

Universal Front Door

Designing a simpler, more joined-up way for people to access Adult Social Care. Aims to reduce duplication and make it easier to get the right support at the right time. Delivery: post 1 April 2027.

Working Age Adults’ New Model of Care

Developing a new approach to supporting working age adults focused on independence, strengths-based practice and reducing reliance on long-term or residential care. Delivery: post 1 April 2027.

Current position (June 2026)

Work is currently focused on scoping, design and early delivery:

  • Initial proposals for the operating model redesign developed
  • Scoping of Universal Front Door and Working Age Adults’ New Model of Care
  • CMS implementation progressing to meet the planned the go-live date

What’s happening next

Over the coming months, the programme will focus on:

  • Developing and refining proposals for future ways of working
  • Moving into phased consultation and implementation where appropriate
  • Continuing delivery of the CMS implementation
