What information do we need?
We only need your unique reference number, known as your Paris ID, your age and gender, along with details of the current care package you are receiving.
Why we need it?
We need this information so we can ensure that you are being provided with the right care to meet your needs.
Our lawful basis for collecting this information is that it is necessary for a task carried out in the public interest or the exercise of official authority carried out by the Council.
In respect of any information which concerns your health, our lawful basis for processing this information is that it is necessary for the provision of health or social care or treatment or the management of health or social care systems and services.
Do we share your information?
Information will be shared between Torbay Council and Torbay and South Devon NHS Foundation Trust, who provide adult social care services on behalf of the Council.
Outside of this, your personal data will only be shared with your consent unless we are required to share your data for law enforcement purposes.
How long do we keep your information?
We will keep your personal data for 3 years after the conclusion of this project. After this time it will be securely destroyed.
