What is a Care Plan anyway?

My Care Plan and the decisions that are made about my care are reviewed regularly. Usually, I have an Independent Reviewing Officer who chairs my reviews and makes sure that my care plan is right for me.

  • Sometimes I don’t feel in control of my care plan…
  • Sometimes I think that people are making decisions about me, and I don’t get a say…

But guess what?

I can chair my own review!

This means:

  • I can invite who I want to be there.
  • I can be the leader of my meeting and ask questions to the people who I invite.
  • I can answer questions about me because I know me best!
  • I can have the review where I want it to be and where I feel most comfortable.

I can talk about things that I want to talk about, like…..

  • Things that bother me.
  • When I’ve felt unwell.
  • What I like to eat.
  • What helps me to stay interested at school
  • What I like to do in my spare time.
  • And what keeps me entertained.
  • When I spend time with my family and friends.
  • Where I’d like to live.
  • And what I would like to do in the future.

I can review my Care Plan because I’m the expert of me!!
