A premises licence can lapse due to the licensee dying, becoming mentally or physically incapacitated, or insolvent. During a period of six months from the date that the licence lapsed, an application can be made to reinstate the Premises Licence.
The application will need to include the following:

Applicants have to pay a fee to apply to reinstate a premises licence.
