What happens next

The following diagrams have been adapted from the Social Care Institute for Excellence (SCIE) website. They show:  

  • the route through assessment  
  • the determination of eligibility  

The first chart shows the stages of completing an assessment: 

We will have a conversation about your situation. During this, we may offer you support that meets your needs without needing an assessment. This could include help from voluntary or community organisations. 

If your assessment confirms you have eligible needs, we will work with you to develop a support plan. This will focus on your: 

  • strengths 
  • existing support network 
  • available community resources 

During the conversation or assessment, we will: 

  • Talk to you about your situation 
  • Discuss things you can do and things you find difficult 
  • Identify support you have and look at other options 
  • Work together to develop a support plan if you are eligible 

You may also be eligible for an assessed amount of funding. You may receive Direct Payments which let you arrange your own care.

There are specific criteria which must be followed to decide if an individual has needs that are eligible for social care services and/or support. These are set out in:

The Care and Support (Eligibility Criteria) Regulations 2015 

If you have care and support needs and a family member or friend supports you, they may be a Carer. They should be involved in your assessment/plans. They are entitled to free support in their own right.
