Support with emotional wellbeing physical health and relationships

Your emotional wellbeing 

We want to make sure your mental health is supported. Many organisations offer help, and your personal advisor can guide you to the right service. This may include adult social care, NHS wellbeing services or funding for therapy, depending on your needs assessment. 

Your physical health 

Your personal advisor can help you manage your health and set goals for physical wellbeing, such as exercising more regularly or giving up smoking.  

Our offer includes access to gym passes and swimming sessions, as well as a list of local resources to support you.  

Having a final health review

Before you were 18, you would have been offered an annual health review. As you approach your 18th birthday, you’ll be invited to a final review. This is important as it helps us support you fully as you move towards independence. 

We understand that talking about your health or attending appointments can be difficult. We will work with you to make it easier. You can ask for a phone call instead of a face-to-face meeting, or let us know if there are topics you would prefer not to discuss. 

This review helps make sure your pathway plan reflects your needs. It can include support with dental care, sexual health, long-term conditions, prescriptions or transport to appointments. 

Your health passport

When you turn 16, you will receive a health passport. This includes information about your health, medical history, treatments and inoculations. We will provide you with an updated version at 18.

Our cared for nursing team are also helping care experienced young people access and use the NHS app. The app lets you view your medical records and request repeat prescriptions, which some people find easier to manage. 

Your relationships 

Everyone deserves to feel cared for and connected. Relationships help us feel safe, supported and part of something. We will support you emotionally and practically to build and maintain positive relationships. You will have access to opportunities to meet new people and strengthen existing connections. 

It’s not always easy to trust others or start new conversations. Your personal advisor will help you with this and support you if relationships become difficult or make you feel confused, unhappy or unsafe. 

We can support you in the following ways: 

  • Help you explore your identity, including your beliefs, culture, sexuality, gender and immigration status. 
  • Support you to understand your experiences, including access to your social care records. 
  • Help you reconnect with family or friends in a safe and planned way. 
  • Provide advice about specialist services, including LGBTQ+ support, healthy relationships and counselling. 
  • Help you stay in touch with people who matter to you and overcome any barriers.

Grants, discounts and allowances for health and wellbeing

Eye care

You may be eligible for free eye tests and vouchers towards glasses/contact lenses if you are: 

  • aged 16 to 18 and in full time education 
  • receiving certain benefits, such as Universal Credit 
  • on a low income and have a Health Credit Certificate (known as HC2 or HC3 certificate). 

Prescriptions

You may be eligible for free prescriptions if you are: 

  • age 16 to 18 and in full time education 
  • receiving certain benefits, such as Universal Credit 
  • pregnant nor have had a baby in the last 12 months (you will need an exemption certificate from your GP) 
  • receiving treatment in hospital. 

We are not able to pay for your prescriptions, but can offer advice and support about prescription fees.
