Information you will need to provide

We want to understand your current situation. We want to know what you need to be well and as independent as possible. We will work with you to understand what is important for you, your hopes and goals, and what help you would need to achieve those. 

Preparing for an assessment 

For a needs assessment, you can think about: 

  • What health conditions do you have and how they affect you
  • What you can do for yourself
  • Who gives you the support and friendship to help you do the things you want
  • How often you need support
  • What is stopping you from getting where you want to be
  • What living arrangements would help you maximise your wellbeing and independence
  • The things that stop you from making things different or better

For a carer’s assessment, you can think about: 

  • The activities you help the person you care for with
  • How many hours a week you provide support
  • Whether you have to help during the day or night, or both
  • If the person you care for has any issues you find hard to deal with
  • Whether your health or wellbeing affected by your caring role
  • If you work, what would make working /caring easier
  • Whether you able to have a break from caring or to plan ahead
  • What would you most like to change about your situation
  • If you want a joint assessment with the person you care for
