ANNEXE 1
MANDATORY CONDITION: WHERE LICENCE AUTHORISES SUPPLY OF ALCOHOL
1) No supply of alcohol may be made under the premises licence:-
() at a time where there is no designated premises supervisor in respect of the premises licence , or
() at a time when the designated premises supervisor does not hold a personal licence or his/her personal licence is suspended.
2) Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
ANNEXE 2
CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
General
1. All staff shall be trained on licensing issues.
The Prevention of Crime and Disorder
1. CCTV shall be installed and maintained to the requirements of the Police.
Public Safety

The Prevention of Public Nuisance
The Protection of Children From Harm
1. A proof of age policy shall be in force and a refusals book maintained.
2. Notices shall be prominently displayed at "Point of Sale" detailing the restrictions on sales to children.
ANNEXE 3
CONDITIONS ATTACHED AFTER A HEARING BY THE LICENSING AUTHORITY
1. CCTV must be used and maintained throughout all times the premises are open to the public. All recordings from that system must be kept for a period of 14 days and the police to have access to recordings at any reasonable time.
The Protection of Children from Harm
1. Any purchase of alcohol by a person who appears to be under the age of 21 years must be asked to provide proof of age and that appropriate signage to this effect must be prominently displayed at the premises.
