Access to an assessment by an Occupational Therapist is via a referral to either the Adult Care Management Team or the Children's Disability Team.
A Referral Co-ordinator will record information and discuss the problems you or the person you are making the call for may be experiencing and will ensure that this information - the 'referral' - is passed to the most appropriate team, or that you are given information to enable you to access independent or voluntary services.
