Help Leaving Hospital

Torbay Care Trust opens in a new window is responsible for providing health and adult social care for people in Torbay.
A small Social Services team is based close to Torbay Hospital. They may be able to provide help or advice for patients, and their families or carers, experiencing difficulties following an admission to hospital. Their main role is to carry out assessments, together with the medical staff, to facilitate safe discharge arrangements.  
If you, or someone you know, is to be admitted to hospital and is over the age of 18, would like help or advice from Social Services, you can ask the Ward to contact the Hospital Social Services Team Referral Co-ordinator at Belmont Court (telephone 01803 654727; textphone 01803 654742).  
If there has already been Social Services involvement and you or the person you are concerned about already has a Care Manager, contact should be made with the Torbay Adults Team, Union House, Torquay (telephone 01803 208500)
